-What We Do-
-A mobile primary care clinic, seeing patients in their homes, Assisted Living, independent living, memory care ect.
-Each patient will get face to face visit monthly with a provider in the comfort of their home.
- Most major insurances providers cover housecalls
- Basis Health partners with local specialist, home health, rehab, mobile labs, pharmacies, home health and hospice so that each patient can have all the additional resources they need.
- Preventative wearable program
With a medical grade wearable device we are able to monitor vitals for our patients 24/7 and intervene with our mobile medical team. Saving the patient a costly stay in the hospital.
We use these devices to monitor and gather data, while leveraging AI technology and predictive models to give us a early warning system, so we can know when a patient is in need of a Basis Health visit.
Where our current healthcare system falls short...
